How do I get started working with Dr. Melissa?
- Please review the services page and these FAQs. Then, fill out the application to work with Dr. Melissa. She will review your information and get back to you with a response within 2-3 weeks.
- If you need progesterone monitoring and/or thyroid monitoring during pregnancy, please fill out the application and call or text us at 972-349-5426 so we can assist you quickly.
What states are included for virtual services?
Texas and Pennsylvania – more states to be added, so please check back!
Does Dr. Melissa accept insurance?
We do not bill insurance or accept Medicare patients. A Superbill (with diagnosis and procedure codes) will be provided after each visit for you to submit yourself for possible reimbursement. The Superbill is for your visit and not lab work. Please note – there is no guarantee on coverage or for reimbursement.
What should I expect at a first visit?
The visit lasts 90 minutes and costs $575. A non-refundable deposit of $150 is required to book the visit and will be applied to the total visit cost. We will review your health history, current concerns, and health goals. We will work together to determine if lab studies, specialized functional medicine testing, and/or a bioresonance scan is needed. An individualized evaluation strategy and treatment protocol will be developed by the end of the visit.
How should I prepare for my first visit?
You will need to complete your intake paperwork and all consent forms on the Cerbo Patient Portal at least 3 business days prior to your appointment. Plan about 1 – 1.5 hours to complete all the forms. This allows Dr. Melissa time to review your information. Please send any medical records for review prior to the visit. If you are charting your menstrual cycle, please send a copy of the chart using the Cerbo Patient Portal prior to the visit.
What should I expect at a follow-up visit?
The visit lasts 45 minutes and costs $300. A package of 3 follow-up visits can be purchased for $825 (savings of $75). All visits in the package must be used within 1 year of purchase. We will review your progress towards your health goals, discuss current concerns, and make any necessary changes to your treatment protocol. Additional testing and evaluations can be ordered as needed. Additional follow-up visits will be scheduled based on your unique situation.
How should I prepare for my follow-up visit?
You will need to complete the Medical Symptom/Toxicity Questionnaire (MSQ) at least 3 business days prior to your appointment. This questionnaire allows us to track your progress overtime. Plan about 10 minutes to complete this. Also, upload any new documents and your most recent menstrual cycle chart (ie: Creighton Model etc) using the Cerbo Patient Portal.
Who do I contact for scheduling or general practice questions?
Please email contact@fiatfertilitywellness.com for scheduling and general practice questions. If you have a patient care related question and are an established patient, please send your question via Cerbo Patient Portal.
How do I send medical records or other documents to be reviewed?
You may send via fax at 972-490-1865 or via Cerbo Patient Portal. You must be an established patient to send medical records or other documents for review. These documents may include, for example, previous lab results, surgery reports, office visit notes, and menstrual cycle charting (ie: Creighton Model etc).
How do I notify Dr. Melissa I am running late for my virtual visit?
Please call or text 972-349-5426.
What do I do for a medical emergency or when I need immediate attention?
Please call 911 or head to the nearest emergency room for evaluation.
What is the cancellation policy?
Your appointment time is reserved just for you! Be courteous and provide notice of at least 24 hours to cancel or reschedule. If there are repeated cancellations, reschedule, or no shows, Dr. Melissa will handle each on a case by case basis. An email or social media direct message does not qualify as a cancellation.
- If your first visit is canceled or rescheduled 24 hours or more before the appointment, your charge will be the $150 non-refundable deposit.
- If your first visit is canceled or rescheduled within 24 hours of the appointment, your charge will be the full appointment fee.
- If your follow-up visit is canceled or rescheduled 24 hours or more before the appointment, there will be no charge.
- If your follow-up visit is canceled or rescheduled within 24 hours of the appointment, your charge will be the full appointment fee.
What types of payments are accepted?
You can pay for visits and testing using credit card, debit card, HSA and FSA.
